PDF forms are made available by the Coast Guard Auxiliary for local reproduction. All forms are linked at the Forms Warehouse available by clicking “PDF Forms” below. E-Forms are submitted via e-mail inside the form.
AUXDATA II is the self-service system to submit activity logs, verify records, and run analytical reports for the Auxiliary. Access to AUXDATA II requires a login and two-factor authentication (2FA). Understanding how to utilize AUXDATA II can be daunting, especially to new Auxiliarists. Contact your servicing Information Services (IS) staff officer for assistance.
Commonly used ANSC and CG forms within the AUP are linked below. It is required that these forms be downloaded to your computer and then be opened and completed using either Adobe Reader or Adobe Acrobat. Attempting to complete a form within a browser will almost always result in the form's appearance and/or functionality to be incorrect.
